Automotive Exhibitor Entry Form

Requirements

  1. Complete a separate entry for each vehicle.
  2. Registration fee is $35 (Includes Goodie Bag).
    ***** NOTE: ENTRY FEE IS NON_REFUNDABLE *******
  3. Application and entry fee must be received no later than April 21st to have your entry on the ballot.
  4. Late registration fee is $45.
  5. Check-in time is from 6:30 to 9:00 AM. Your entry packet will be at the check-in and entry at Conejo Creek Park South, Hwy 23 at Janss Road.

In consideration of acceptance to the Conejo Valley Classic Car Show, I or my representative hereby fully and unconditionally agree to:
  • Release the Conejo Valley Rotary Club, Thousand Oaks Rotary Club, Conejo Park & Recreation and City of Thousand Oaks and any of their representatives from any liability that may arise out of the April 25, 2010 Classic Car Show;
  • Release all officials, personnel, sponsors or co-sponsoring businesses or individuals, jointly and separately, from any and all liability arising out of said event
  • Does hereby give permission to use information of entrant's automobiles for publicity purposes.
